  5. ABDM will be designed to monitor and display the performance and accountability of all health service providers in real-time. This performance evaluation will apply to health institutions and professionals, based on agreed Key Performance Indicators (KPIs) at various service levels within the health sector.
  6. ABDM will have a nationwide reach and facilitate seamless portability across the country through a Health ID, which serves as a Personal Health Identifier. Adopting Health Information Standards will play a pivotal role in achieving national portability.
  7. The ABDM ecosystem will follow the principle of “Think big, start small, scale fast.” It will employ a combination of strategies, including minimalistic design, prioritization, and agile technology architecture to rapidly scale both horizontally and vertically.

Technology Principles

  1. ABDM will be developed following the India Enterprise Architecture Framework (IndEA), conforming to the Agile IndEA Framework as notified by the Ministry of Electronics and Information Technology (MeitY).
  2. All components of ABDM will conform to open standards, ensuring interoperability and relying on open-source software products and open-source development. The policy on Open Standards and Open Source Software, as notified by MeitY, will be adopted in designing the components and in all procurement activities.
  3. ABDM will adopt a Federated Architecture, where only the core building blocks will be developed and maintained centrally. Other building blocks will operate in a federated model, allowing regional, state-level, and institution-level platforms and systems to function independently but in an interoperable manner. Data will be federated and stored close to its point of generation.
  4. ABDM will be an Open API-based ecosystem, adhering to the Open API Policy as notified by MeitY. Data sharing will follow standards defined in NDHB, with a strong emphasis on security and privacy during API design and development.
  5. Legacy systems will be assessed for compliance with NDHB principles and integrated into ABDM when feasible. Only legacy systems meeting the required standards will be allowed within the ecosystem. Valuable data from legacy applications about healthcare providers, labs, and patients will be leveraged to reduce the effort of collecting such information.
  6. All components, building blocks, registries and artefacts of ABDM will be designed with a minimalistic approach in mind to encourage broad adoption.
  7. Registries and master databases within ABDM will be developed as Single Sources of Truth on different aspects, backed by robust data governance. Strict validation processes, clear ownership and responsibilities, and dedicated data governance structures at the state and central levels will be established.

Building Blocks of ABDM

  1. Health ID: The process of identifying individuals across healthcare providers will be standardized. This will ensure that medical records are issued to the correct individuals or accessed by Health Information Users with the appropriate consent. The Health ID will serve as a unique identifier for individuals and will be used for authentication and linking health records across various systems and stakeholders.
  2. Healthcare Professionals Registry (HPR): A comprehensive repository of all healthcare professionals involved in delivering healthcare services, spanning modern and traditional systems of medicine. Enrolling in the HPR will connect healthcare professionals to India’s digital health ecosystem.
  3. Health Facility Registry (HFR): A comprehensive repository of health facilities across India, encompassing various systems of medicine, including public and private health facilities such as hospitals, clinics, diagnostic laboratories, imaging centres, and pharmacies. Enrolling in the HFR will connect these facilities to India’s digital health ecosystem.
  4. ABHA Mobile App (PHR): The ABHA mobile application serves as a Personal Health Record (PHR), an electronic record of an individual’s health-related information adhering to nationally recognised interoperability standards. Individuals have control over their health information, allowing them to manage and view a longitudinal record comprising health data, lab reports, treatment details, and discharge summaries from multiple healthcare facilities. The app provides features such as creating an ABHA address, discovering health information, linking health records, viewing health records and managing consent.
