Then you must input the Member from the list and Date Of Birth. Enter your passport number in the Enter Passport Number section. Tick the declaration I declare that this passport belongs to the beneficiary. The name of the passport holder is the same as mentioned on the vaccine certificate. Ensure that the given passport number is correct and confirm passport details Click on SUBMIT REQUEST. 8. Go to Account Details and click on International Travel Certificate. 9. You will now be able to download your international vaccine certificate linked to your passport.
