To register for the Aadhaar, individuals must submit a duly filled Enrolment Form along with the required supporting documents. Eligible residents can use this link to download a free copy of the Aadhaar card Enrolment Form from the UIDAI website. The main contents of the Enrolment Form are as follows;

  1. Pre-Enrolment ID, NPR Receipt or TIN Number
  2. Full Name
  3. Gender of the applicant
  4. Age of the applicant
  5. Residential Address
  6. For children below the age of five, details of their Father/Mother/Guardian 
  7. Details of submitted supporting documents
  8. In case they do not have the required documents, Introducers or Head of Family’s details
  9. Applicant’s Signature or Thumb imprint
  10. Date and time of enrolmentAadhaar enrolment form

How to fill the Aadhaar Card Enrolment Form

  1. Make sure that all the details you provide are the same as on the supporting documents that you are submitting.
  2. If you are applying for a child below the age of five years, make sure you attach a copy of the child’s parent’s Aadhaar.
  3. Register your mobile number and email ID as soon as possible as the linking of the same is required for several Aadhaar services.
  4. Users can make corrections to their form within 96 hours without making any additional payment.
  5. To make corrections, you will have to fill the enrolment form again. After 96 hours, users will have to pay a fee of INR 25 to make changes to their Aadhaar details.
